Responsabilités

• Ensure reablement goals/outcomes are met according to the reablement plan.
• Identify additional goals to improve quality of life and promote independence.
• Provide feedback to the Reablement and Assessment Officer.
• Maintain clear and timely communication with the Reablement and Assessment Officer.
• Work closely with a small team of reablement support workers to track progress.
