Job summary

West Northamptonshire Council is seeking Reablement Support Officers to work in the community, travelling to clients’ homes to support them in regaining daily living skills and independence with dignity.

You will work a rotating shift pattern (4 days on, 4 days off) averaging about 31.5 hours per week, with morning and evening shifts, and you will be paid for whole shifts with 45p per mile.

Qualifications

• Experience in health or social care settings.
• Willing to work towards NVQ level 2 in Health & Social Care or equivalent.

Responsibilities

• Visit clients in their homes to help regain skills and confidence.
• Case manage Reablement journeys from initial assessment to discharge.
• Signpost to other agencies and coordinate with health partners.
• Monitor progress and support planned Reablement goals.
• Work countywide across Northamptonshire and manage rota with rotating shifts.
