Job summary

Walsall Council's Intermediate Care Service is seeking Reablement Officers to support customers in their own homes and deliver active reablement plans within a multidisciplinary team.

You will work part-time (30 hours per week) and help customers regain skills, coordinate activities of daily living, and promote independence in the community, in line with personalised support plans. Minimum qualification NVQ Level 2 in Health and Social Care is required.

Qualifications

• Experience in community-based Reablement Services.
• Experience of working with customers in their own homes.
• Understanding of strength-based working approaches.
• Excellent written and verbal communication skills.

Responsibilities

• Support customers in their own homes to develop/regain skills and confidence.
• Coordinate and deliver a programme of active Reablement to enhance independence.
