Mersey Care NHS Foundation Trust is seeking a Band 3 Reablement Worker to join the Longmoor House team at the Aintree Hospital site. The role focuses on delivering rehabilitation and reablement services to help patients regain independence and return home, working within a multidisciplinary team on a 24/7 shift pattern.

The post requires experience in caring roles, NVQ 3 health-related work, and GCSE-level literacy and numeracy.

Qualificações

• Written, verbal and numeric skills to GCSE or similar level.
• NVQ Level 3 health related study or equivalent experience.
• Care Certificate (desirable)

Responsabilidades

• Deliver clinical and therapeutic care to patients as per care plan and assist the nurses and Allied Health Professionals.
• Undertake rehabilitation and reablement and report to the multidisciplinary team plan.
• Perform patient assessment to identify goals and plan and deliver high standards of care.
