Job summary

Hoople Group in Hereford is seeking an Assessment and Review Officer to join our Home First team. You will assess adults recovering from illness or injury, creating tailored plans that support independence and safe home life.

Qualifications

• Experience of assessing adult care needs, risk management and care planning.
• Background in social care, health care or community settings.
• Excellent communication, problem-solving and decision-making skills.

Responsibilities

• Complete timely assessments, reviews and support plans reflecting needs, goals and strengths.
• Coach and empower people to engage in their own recovery using a reablement approach.
• Assess and manage risk, ensuring safe, person-centred care.
• Work with families, carers and partner agencies to deliver effective support.
• Support deployment of care staff and ensure a high-quality, CQC‑compliant service.
• Teach and demonstrate mobility aids and assistive technology.
• Maintain accurate electronic records and documentation.
