Job summary

Derbyshire Community Health Services NHS Foundation Trust is looking for a Rehabilitation Support Worker to join our Rapid Response Team, delivering rehabilitation alongside the therapy group in community settings. You will support patients at home and in care homes, creating patient-led care plans and contributing to discharge planning with the multidisciplinary team.

A weekend roster to respond to rapid therapy visits is included.

Qualifications

• Willingness to undertake further recognised training programmes applicable to the role.
• Good standard of general education, including English Language and Maths GCSE or equivalent.
• NVQ Level 4 in health or social care, or able to demonstrate equivalent level of knowledge/skills gained through experience and training/development
• Assistant Practitioner Qualification or equivalent

Responsibilities

• Initial assessment and reviews of patients; create patient-led care plans.
• Communicate health needs and treatment plans to patients and carers.
• Maintain health records using System One/TPP; ensure accuracy.
• Respond to Rapid Therapy visits on a weekend roster.
• Organise and prioritise day-to-day work and follow lone-working policies.
