Job summary

Connected Health Group Limited is seeking a Rapid Responder to support safe, high‑quality home care services across the Trust zone. You will work with the Registered Manager and Expansion Team to launch new services, assist hospital discharges and fill care packages.

The role requires flexibility and a commitment to excellent care, with duties including delivering PPE, care plans and essential documentation to ensure smooth service delivery.

Qualifications

• Minimum of 6 months' homecare experience.
• Excellent verbal and written communication in English.
• Full clean driving licence.
• Access to a car for work purposes.
• Ability and willingness to travel throughout the allocated Trust zone.
• Strong organisational and time management skills.
• Flexible approach to working hours and service needs.
• Ability to work closely with the Registered Manager to support new and developing services.

Responsibilities

• Support the development and mobilisation of new home care services.
• Provide cover across the allocated Trust zone and assist with hospital discharges.
• Deliver care plans and log sheets to clients' homes as required.
• Deliver PPE and essential documentation or supplies to clients and staff.
• Check and collect daily record sheets from clients' homes.
• Travel between clients' homes, offices and service areas within the zone.
