Dorset HealthCare University NHS Foundation Trust is seeking an Urgent Community Response therapist part-time (23 hours per week) to deliver rapid assessments and short-term interventions in patients' homes, working with a multidisciplinary team across Integrated Community Services and Integrated Urgent Care.

The role requires autonomous decision-making, strong communication, and recent experience in rapid response or urgent care. DBS check is required and secondment arrangements may apply.

Qualifikationen

• Registered Allied Health Professional (AHP on HCPC register).
• BSc (Hons)/Diploma in Physiotherapy or Occupational Therapy.
• Full UK drivers' licence.

Aufgaben

• Provide 2-hour urgent health response in patients' homes.
• Assess and implement short-term interventions.
• Collaborate with 111, 999, and community teams for patient-focused care.
• Support families and carers and contribute to cross-site coverage.
