NELFT North East London Foundation Trust is recruiting for a Hybrid Worker/Nurse Associate to join the Rapid Response Team. The role focuses on delivering urgent community-based care, reducing unnecessary hospital admissions, and supporting patients in their own homes.

You will work under supervision, undertake delegated assessments, contribute to care planning, and coordinate care packages while promoting patient independence and collaborating with multidisciplinary teams.

Formación

• NVQ level 2 or equivalent standard of literacy and numeracy
• Nurse Associate Foundation Degree
• Foundation Degree or equivalent nursing qualification

Responsabilidades

• Organise and prioritise own workload in day-to-day tasks.
• Carry out home assessments within scope and guidelines.
• Contribute to and deliver care planning in patients' homes.
• Manage rapidly changing caseload with senior support.
• Adhere to Trust protocols, including med admin and moving handling.
• Maintain accurate records for audit and care interventions.
• Respond quickly to emergencies in the community.
