Job summary

NELFT NHS Foundation Trust is seeking an Assistant Practitioner (Nursing Associate qualified) to join the Rapid Response Team, delivering urgent community-based care to patients in their own homes. You will work under supervision to undertake delegated assessments, support care planning, and provide holistic clinical care while collaborating with Registered Nurses, GPs, therapists and social care teams.
