Job summary

Choices Homecare, part of the Optimo Care Group, is recruiting Rapid Response Care/Support Workers for clients across the Kirklees region in the UK. The role requires compassionate, reliable individuals with strong communication and teamwork skills to support service users in community settings.

Responsibilities include initial assessments, care planning documentation, personal care, medication support, mobility assistance, and collaboration with healthcare professionals, while upholding dignity

Qualifications

• Level 3 Health and Social Care qualification or equivalent.
• At least 2 years' experience in Health and Social Care.
• Good written English skills.

Responsibilities

• Provide appropriate support to service users during initial assessment.
• Gather information and complete documentation to support the care plan.
• Carry out initial care calls to establish the service user on their routine care round.
• Undertake risk assessments identifying potential or immediate risks.
• Provide personal care (washing, dressing, toileting, continence).
• Support with medication following instructions and procedures.
• Assist with mobility and transfers using equipment and safe techniques.
• Help with meals, hydration and basic household tasks.
• Provide emotional support during illness, crisis or change.
• Monitor and report health or wellbeing changes to professionals.
• Collaborate with nurses, social workers, OT, GPs and others.
• Promote independence, dignity and person-centred care.
• Maintain confidential records and report safeguarding concerns.
• Follow health and safety, infection control, safeguarding and confidentiality policies.
• Adapt to changing priorities and work under pressure calmly.
• Support people to remain at home and prevent unnecessary hospital admissions.

About the Role

A Rapid Response Care/Support Worker should demonstrate compassion, reliability, communication skills, flexibility, observation skills, teamwork, resilience and the ability to work independently in a fast-paced environment.

Key Responsibilities

• Provide appropriate support to service users during their first call or initial assessment, ensuring their needs, preferences, and circumstances are clearly understood.

• Gather all relevant information and complete or obtain the necessary documentation and paperwork to support the development and implementation of an effective care plan.

• Carry out care calls as required during the initial period of service, providing continuity and reassurance until the service user is successfully established on their regular care round.

• Undertake appropriate risk assessments, identifying potential or immediate risks to service users, staff, and others.

• Provide personal care, including washing, dressing, toileting and continence support.

• Support with medication, following prescribed instructions and organisational procedures.

• Assist with mobility and transfers, using appropriate equipment and safe moving-and-handling techniques.

• Help with meals, hydration and basic household tasks where required.

• Provide emotional support and reassurance, particularly during periods of illness, crisis or change.

• Monitor and report changes in a person’s health, wellbeing or behaviour to the appropriate professional.

• Work closely with nurses, social workers, occupational therapists, GPs and other health and social-care professionals.

• Promote independence, dignity, choice and person-centred care.

• Maintain accurate, confidential records and report incidents or safeguarding concerns promptly.

• Follow health and safety, infection control, safeguarding and confidentiality policies.

• Manage changing priorities and work effectively under pressure while maintaining a calm and professional approach.

• Support people to remain safely at home and, where possible, prevent unnecessary hospital admission or facilitate a safe discharge from hospital.

Essential experience and skills

Level 3 or equivalent in Health and Social Care

At least 2 year experience working in a Health and Social Care environment

Good Written English Skills
