Job summary

Choices Homecare, part of the Optimo Care Group, is recruiting Rapid Response Care/Support Workers to provide care across the Wakefield and Kirklees region. The role focuses on compassionate, responsive support and working closely with health and social care professionals.

Key duties include initial assessments, personal care, medication support, mobility assistance, meals and hydration help, and maintaining accurate records.

Responsibilities

• Provide appropriate support to service users during initial calls or assessments.
• Gather information and complete documentation to develop and implement care plans.
• Carry out care calls to ensure continuity until the regular care round is established.
• Undertake risk assessments and monitor safety for service users and staff.
• Provide personal care, including washing, dressing and continence support.
• Support with medication following procedures.
• Assist with mobility and transfers using proper techniques.
• Help with meals, hydration and basic household tasks.
• Provide emotional support during illness or crisis.
• Monitor changes in health or wellbeing and report to professionals.
• Collaborate with nurses, social workers and other professionals.
• Promote independence, dignity and person-centred care.
• Maintain confidential records and report safeguarding concerns.
• Follow health, safety, infection control and confidentiality policies.
• Adapt to changing priorities and work under pressure while staying calm.
• Support people to remain safely at home and aid discharge when possible.
