Key Responsibilities
  • Complete Care Act Assessments for young people transitioning to adult services.
  • Develop and implement personalised Support Plans.
  • Arrange, monitor, and review support packages to ensure positive outcomes.
  • Undertake safeguarding enquiries in line with statutory duties.
  • Work in partnership with Children's Services, Education, Health, and other stakeholders.
  • Promote independence and wellbeing through strengths-based practice.
  • Maintain accurate case records and ensure compliance with legislation and policies.
Essential Requirements
  • SWE registered with a recognised Social Work qualification (CQSW, DipSW, or equivalent).
  • Previous experience within Transitions, Adult Services, or Children's Services.
  • Strong knowledge of the Care Act 2014, safeguarding frameworks, and statutory responsibilities.
  • Ability to work collaboratively with multi-agency partners.
  • Right to work in the UK.
Location and Working Pattern

Based within the Royal Borough of Kingston upon Thames Transitions Team, with hybrid working available depending on service needs.
