Job summary

Lincolnshire County Council’s Learning Disability South Team is seeking a committed Learning Disability Nurse or Social Worker to work 19 hours per week (Wed–Fri) in Sleaford/Spalding. You will complete Care Act assessments, develop and review support plans, and supervise up to four staff in a person-centred, strengths-based team.

This community-based role requires current Nursing or Social Work registration, willingness to travel for visits, and the ability to contribute to safeguarding, CHC

Qualifications

We welcome qualified Learning Disability Nurses or Social Workers with current registration.
Community-based role requiring travel to visit adults in Lincolnshire.
Ability to supervise up to four staff and support team development.

Responsibilities

Complete Adult Needs Assessments and reviews under the Care Act 2014.
Develop and review strengths-based, outcome-focused support plans.
Collaborate with individuals, families, carers and partner agencies to promote independence and wellbeing.
Lead on Section 75 reviews/Decison Support Tool and CHC checklists where appropriate.
Supervise and mentor junior colleagues within the Learning Disability South Team.
