Qualifications

• Newly qualified or experienced Social Worker to join the Children with Disabilities (CWD) Team.
• Experience with children with disabilities would be beneficial, but training and development opportunities are provided.

Responsibilities

• Manage a range of cases from assessment to ongoing interventions, leading on these from start to finish.
• Assess social care needs, support planning and review packages of care, including transitions to adult care.
• Build and maintain long-term relationships with children, young people and families, working with multi-agency partners.
