Job summary

Central London Community Healthcare NHS Trust is recruiting Neighbourhood Case Coordinators to join the Hertsmere Proactive Care Service. The role blends clinical visits and administrative tasks to support a cohort of residents with frailty, coordinating care across health, social care and voluntary services.

You will visit patients at home or in clinics, carry out assessments, develop preventative care plans, and participate in MDT meetings.

Qualifications

• Hybrid clinical/administrative role.
• Visiting patients at home or in clinics to assess long-term conditions and provide advice.
• Full UK driving licence and access to a car for business use.

Responsibilities

• Support patients on Proactive Anticipatory Care Service with care plans.
• Arrange and attend meetings with health and social care colleagues.
• Provide clinic letters, MDT minutes, and appointment bookings.
