Job summary

South Road PCN is seeking a part-time LTC Proactive Care Nurse to assess, plan and manage patients with complex long-term conditions across multiple sites. The role includes home visits, care home visits and close collaboration with GPs, nurses and community services.

You will lead holistic assessments, support self-management and contribute to reducing hospital admissions. The ideal candidate is an experienced, NMC-registered nurse with expertise in diabetes, cardiovascular and respiratory

Qualifications

• NMC Registered Nurse with current UK registration.
• Experience in managing long-term conditions in primary care.
• Competence in diabetes, cardiovascular and respiratory care.

Responsibilities

• Assess, plan and implement care for patients with complex LTCs.
• Undertake home visits and care home visits across the PCN footprint.
• Coordinate care with GPs, practice nurses, pharmacists and social care teams.
