Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking a Proactive Care Lead to join the neighbourhood team and drive a preventative, person-centred approach to care for people with complex needs. You will coordinate care across primary care, community, acute, mental health and voluntary sector services to support independence and reduce unnecessary hospital admissions.

The successful candidate will be an experienced clinician with strong communication and leadership skills,

Qualifications

• Registered healthcare professional (RGN, HCPC or equivalent professional registration).
• Independent prescribing and Advanced Health assessment qualification.
• Degree-level qualification or equivalent experience.
• Evidence of continuing professional development.
• Leadership and management qualification or equivalent experience.
• Master's level study or working towards Master's level qualification, or equivalent experience.
• Training in quality improvement, service redesign, population health management or personalised care.

Responsibilities

• Provide leadership for proactive care across the neighbourhood, working with multidisciplinary teams and partner organisations to identify and support patients with complex needs.
• The post holder will coordinate personalised care, lead service improvement, promote integrated working and improve outcomes through a preventative, person-centred approach.
