Oxleas NHS Foundation Trust is seeking a Prisons Services Clinical Pharmacist to optimise medicines support for HMP Elmley on Sheppey. You will lead medicine reviews, stratify risk with high‑risk medicines, and support the prison formulary alongside the pharmacy team.

As part of the role, you will drive audit activities, ensure compliance with NICE guidance, and contribute to education for the primary care team and dispensary staff across the trust.

Responsabilidades

• Support patients with their medicines queries and concerns.
• Undertake scheduled medicine review clinics and SMRs.
• Identify high‑risk medicines and implement risk reduction measures.
• Develop and manage the prison formulary with OPS colleagues.
• Lead audits and disseminate findings to improve prescribing practice.
