Job summary

Change Grow Live in Sefton is seeking a Prison In-Reach Recovery Coordinator to assess and engage with service users, coordinating resettlement plans from prison and liaising with partner agencies to address substance misuse and offending behaviour.

You will build strong links with prison security and community services, advocate for service users, and support social reintegration while promoting recovery and resilience. This full-time role requires DBS check and security clearance.

Qualifications

• Ability to obtain and maintain prison security clearance.
• Strong understanding of substance misuse and similar field.
• Excellent verbal and written communication with good IT skills.
• Proactive with strong time management and prioritising.
• Knowledge of anti-social behaviour legislation.
• Experience with people with offending backgrounds incl. MAPPA.
• Good working knowledge of mental health interventions.
• Ability to work with professionals, agencies and stakeholders.
• Ability to manage change prioritising service users.
• Knowledge of the criminal justice system.

Responsibilities

• Coordinate comprehensive assessments and resettlement plans from prison by liaising with relevant agencies.
• Act as navigator and advocate for service users.
• Reduce drug and alcohol related harms and promote involvement of carers, service users and community.
• Provide advocacy for access to partnership services.
• Support social reintegration and promote recovery, resilience and self-determination.
• Carry out assessments, map resources and develop action plans addressing offending/substance use.
• Collaborate with prison security and partner agencies within the system.
• Work with the team to identify individuals with substance misuse and reoffending histories.
