Job description

We believe in treating everyone as an individual, providing support with respect and care, and working together to create meaningful change.

We are looking for a Prision In-Reach Recovery Coordinator to join our team in Coventry. Your role will focus on identifying, engaging and supporting service users with drug and/or alcohol needs, delivering targeted interventions and robust release planning to reduce substance misuse, re-offending and associated harm. You will work closely with prisons, probation services, and community-based providers to ensure seamless transitions and timely access to appropriate support on release.

A fantastic oportunity has arisen to become a Prison In-Reach Coordinator on a permanent basis, engaging service users who are engaged in a cycle of reoffending. This role involves weekly travel to our feeder prison, HMP Hewell, therefore access to a car and valid clean driving licence is essential for this role.

  • Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
About The Role
  • Act as a key liaison between prison in-reach teams and community drug/alcohol services to support continuity of care, shared case management, and coordinated risk reduction planning.
  • Carry out prison-based screenings, initial assessments, and risk assessments, developing recovery plans in partnership with service users prior to release.
  • Provide harm reduction education, overdose prevention, and relapse management planning with service users in custody and following release into the community.
  • Deliver and promote Naloxone provision pre- and post-release, and train service users and peers in its use where appropriate.
  • Offer “through-the-gate” support including gate pickups where appropriate and facilitate access to community recovery services, housing, and health appointments upon release.
  • Conduct home visits, hostel visits, and community outreach as needed to maintain engagement, particularly with high-risk individuals and those at risk of disengagement.
  • Work collaboratively with police, probation, mental health, housing, and social care partners to address the complex needs of high-risk service users and promote recovery and rehabilitation.
About You
  • Empathy and Understanding: A strong grasp of substance misuse issues and experience working in similar fields.
  • Clear Communication: Excellent verbal and written communication skills, along with proficiency in IT.
  • Self-Motivated and Organised: Ability to take initiative, manage time effectively, and prioritise tasks.
  • Knowledgeable and Resourceful: Good understanding of mental health interventions and services.
  • Team-Oriented: A supportive team player with strong interpersonal skills, comfortable collaborating with a range of professionals and stakeholders.
  • Adaptable: Ability to manage change effectively, always prioritising the needs of our service users.
  • Committed to Best Practices: Understanding of information governance processes and dedication to applying all necessary safeguards.
  • Continuous Learner: Enthusiastic about seeking out learning opportunities and contributing to the development of others.
  • Approved for Restricted Institutional Access: The successful candidate will need to complete an enhanced DBS check and obtain HMPPS security clearance
What We Offer
  • Wellness: Enjoy a paid ‘Wellness’ hour each week, access to a ‘Wellness’ hub, and an Employee Assistance Programme.
  • Time Off: Generous annual leave entitlement (+ bank holidays), increasing yearly for the first five years.
  • Financial Benefits: A contributory pension scheme and discounts on shopping, cinema, holidays, and more.
  • Refer a Friend Scheme: receive a voucher when they successfully join our team!
