The post-holder will be given opportunities to shadow other healthcare professionals and be supported to develop and advance the role within the PCN pharmacy team, working both autonomously and in collaboration with other members of the wider multidisciplinary team.

We are passionate about developing the pharmacy professionals of the future, and the successful candidates will be supported to enroll onto the 15-month CPPE Primary Care Pharmacy Education Pathway, in addition to joining a peer-to-peer mentoring programme under the supervision of a consultant pharmacist to further support education and development.

Main duties of the job

The post-holder will be responsible for carrying out a variety of duties and roles for individual practices and the PCN. This includes driving medicines reconciliation and optimization through tasks such as the safe transfer of care from secondary and tertiary settings, promoting and managing repeat dispensing and online ordering to reduce medicines wastage through safe and cost-effective prescribing.

The post-holder will also answer medication queries from clinicians, colleagues, and patients within their scope of practice and competency, referring to clinical pharmacists as appropriate. There will be opportunities to run face-to-face clinics managing patients with chronic conditions and actively review and optimize their medication.

About us

We are a dynamic and supportive PCN, serving a diverse cohort with various demographics and health conditions, including a large student population.

The successful candidate will join a well-established, innovative pharmacy team of five clinical pharmacists and three pharmacy technicians, supported by a lead PCN pharmacist.
