Join us in supporting Cranbrook Urgent Primary Care, where our interdisciplinary team provides a range of services in a coordinated approach to healthcare. Primary Health Care ensures clients receive the right care by the right provider, when and where they need it. Work days may be Monday to Friday from 08:30 to 16:30 on a casual, as-needed basis; hours are not guaranteed and may vary depending on staffing needs.

About the position:

Primary Care Centres are community-based networks of healthcare professionals and clinics that plan and deliver day-to-day healthcare needs. Teams work closely to provide better care, by the right person, at the right time, closer to home.

In this role, the Physiotherapist collaborates with an inter-professional care team and a network of PCN Physicians, Nurse Practitioners, Nursing, and other Allied Health members. Responsibilities include integrated care planning, client assessments, clinic visits, ongoing monitoring, and self-management support for clients (adults, seniors, pediatrics), including those with multiple chronic conditions, in individual or group settings, often within Physician/Nurse Practitioner clinics.
