Job description

Join Us in Transforming Cardiovascular Care in Primary Practice

We are seeking an enthusiastic and experienced Practice Nurse who is passionate about the management of Cardiovascular Disease (CVD) to join our growing Primary Care Network (PCN) team, working across three GP practices. This role is ideal for a nurse who wants to help shape proactive, preventative care at the heart of local communities.

This role is for a minimum of 22.5 hours per week up to 37.5 hours per week.

Main duties of the job

This role focuses on supporting patients who have or are at risk of cardiovascular disease across three GP practices within the PCN, to help deliver high-quality preventative care, health promotion, and long-term condition management. You will work both independently and collaboratively within a multidisciplinary team to improve cardiovascular outcomes and reduce health inequalities.

As a core member of our multi-disciplinary team, you will contribute to the ongoing management and health promotion of patients with cardiovascular risk factors and conditions including hypertension, atrial fibrillation, and heart failure.

You must be a registered Nurse (NMC) with at least 18 months post-registration experience, with knowledge of cardiovascular disease and prevention.

The Brunel Primary Care Network consists of three Practices across Portsmouth: East Shore Partnership, The Lighthouse Group, and University Surgery.

We have highly skilled and motivated teams working within our PCN, which include GP Partners, Salaried GPs, Advanced Nurse Practitioners, Pharmacists, Diabetes and Respiratory Nurse Specialists, Nurses, GP Assistants, and HCA's, supported by skilled administration teams.

Job responsibilities

Key Responsibilities

  • Clinical Care
  • Provide nurse-led reviews for patients with hypertension, AF, heart failure, and related conditions.
  • Carry out ECGs, BP monitoring, lifestyle risk assessments, and QRISK scoring.
  • Deliver lifestyle advice and structured education to improve cardiovascular health.
  • Document all assessments and care plans using EMIS / SystmOne.
  • Teamwork and Integration
  • Collaborate with GPs, clinical pharmacists, social prescribers, and care coordinators.
  • Attend PCN-wide team meetings and contribute to joint patient care planning.
  • Promote consistent approaches across all three practices and contribute to population health strategies.
  • Professional Development
  • Act as a role model for patient-centred, evidence-based care.
  • Engage in ongoing professional development focused on cardiovascular and chronic disease care.
  • Support peer learning and mentorship for junior colleagues or students.
  • Quality, Safety and Data
  • Follow all clinical governance, safeguarding, and infection control policies.
  • Participate in audits and contribute to QOF/IIF achievement.
  • Collect and interpret clinical data (e.g., BP, lipid levels, QRISK) to support proactive case finding and care planning.
  • Identify and escalate any clinical or population-level risks through appropriate governance structures.
  • Contribute to the development of data-informed approaches to improving patient outcomes across the PCN.
Person Specification
Qualifications
  • NMC Registered Nurse with a minimum of 18 months post-registration experience.
  • Additional training in cardiovascular or chronic disease management.
Experience
  • Experience in collating clinical data and recognising risk trends in patient care.
  • Primary care or community experience.
  • Involvement in audits or service evaluation.
Skills
  • BP monitoring, ECGs, lifestyle advice, documentation.
  • Able to interpret clinical trends and respond appropriately to risk.
  • Understanding of QOF/IIF frameworks.
  • Familiarity with clinical dashboards or population health tools.
  • Interest in developing advanced skills in CVD.
