Cheshire & Wirral Partnership NHS Foundation Trust is seeking a 12-month fixed term Care Coordinator to join our Administration/Reception Team. The role is part time (28.5 hours per week) and covers Westminster Surgery and Old Hall Surgery, supporting population health initiatives and PCN service specifications through proactive patient coordination.

You will use GP practice systems (EMIS), identify cohorts, facilitate self-management education, connect patients with peer support, and coordinate

• Foundation degree or equivalent level qualification.
• Understanding NHS primary care processes.
• Ability to prepare concise communications.
• Experience coordinating projects in complex environments.
• Experience with data collection and monitoring.
• Strong collaboration across teams and partners.
