Meon Health Practice is seeking a motivated Practice Nurse to join our busy, supportive team across three sites. This role offers variety, ongoing professional development and the chance to make a real difference to patients’ lives.

You will work alongside GPs and the wider clinical team in a collaborative environment. You will provide a range of general practice nursing services, undertake chronic disease reviews, immunisations and wound care, and maintain accurate, confidential patient records

Qualifikationen

• Experience working in Primary Care or the Community.
• Current NMC registration.
• Good clinical assessment and decision-making skills.
• Ability to work safely and effectively as part of a multidisciplinary team.
• Good communication and interpersonal skills.
• Commitment to providing high-quality, patient-centred care.
• Ability to maintain accurate and confidential patient records.
• Ability to manage workload effectively and work independently when required.
• Commitment to continuing professional development.
• Understanding of professional responsibilities and the NMC Code.
• Experience using GP clinical systems.
• Relevant Practice Nursing or Primary Care qualifications.

Aufgaben

• Provide a range of general practice nursing services to patients.
• Undertake chronic disease reviews and support ongoing management of long-term conditions.
• Carry out immunisations, vaccinations and health promotion activities.
• Provide treatment-room services, including wound care, dressings and other appropriate clinical procedures.
• Support the delivery of practice clinical targets and quality improvement initiatives.
• Work collaboratively with GPs, nurses, healthcare assistants and other multidisciplinary team members.
• Maintain accurate, timely and confidential patient records in accordance with practice procedures and professional standards.
