Job summary

NHS Jobs is seeking an Experienced Practice Nurse for a primary care setting. The role focuses on developing and embedding health promotion and well-being programmes, delivering routine nursing care, and supporting patients with chronic conditions in accordance with NICE, NSF and local policies.

You will implement and evaluate care plans, ensure QoF/QiF adherence, assist with minor procedures, vaccinations and travel health services, and collaborate with GPs and external services to provide

Responsibilities

• Develop, implement and embed health promotion and well-being programmes.
• Implement and evaluate individual treatment plans for chronic disease patients.
• Ensure QoF & QiF adherence to meet targets.
• Identify, manage and support patients at risk of developing long-term conditions.
• Provide routine nursing care in accordance with clinical evidence, NICE, NSF and guidelines.
• Provide wound care and vaccination/advisory services as required.
• Maintain accurate records and ensure read codes are used effectively.
