Job summary

Operose Health is seeking a Practice Nurse for Kirkby Community Primary Care in Nottingham. The role focuses on delivering high-quality primary care with emphasis on LTC management and patient experience.

You will support the registered manager, collaborate with the practice team on chronic disease management, treatment, and care evaluation, while ensuring adherence to policies and CQC standards. 7.5 hours per day with a salary up to £48,000.

Qualifications

• Long Term Condition (LTC) experience essential.
• UK registered nurse with significant post registration experience.
• Experience in cervical cytology and baby immunisations.
• Good knowledge of the Quality Outcomes Framework (QOF).

Responsibilities

• Assist the registered manager with duties related to CQC outcomes and ensure policy compliance.
• Provide care for presenting patients, including treatment, chronic disease management and evaluation of care.

Role Responsibility

As a valued member of the Operose Health team, your key duties will include:

• You will assist the registered manager with duties related to CQC outcomes, ensuring that compliance is maintained. As well as working collaboratively with the general practice team to meet the needs of the patients, ensuring practice policies and procedures are followed.
• Providing care for the presenting patient, treatment, chronic disease management and evaluation of care.
