Job summary

Operose-Health in Nottingham invites a Practice Nurse to join Kirkby Community Primary Care. You will deliver care, support chronic disease management and work with the team to improve access, outcomes and patient experience.

The ideal candidate is a UK registered nurse with LTC experience, cervical cytology and baby immunisations experience, and a solid knowledge of the Quality Outcomes Framework. Salary up to £48,000 depending on qualification and experience.

• UK registered nurse with significant post registration experience.

Main Responsibilities:

• You will assist the registered manager with duties related to CQC outcomes, ensuring that compliance is maintained. As well as working collaboratively with the general practice team to meet the needs of the patients, ensuring practice policies and procedures are followed.
• Providing care for the presenting patient, treatment, chronic disease management and evaluation of care.
