Job summary

The Uplands Medical Practice is seeking an experienced Practice Nurse to join our friendly multidisciplinary team for three days per week. You will deliver high-quality nursing care in a busy GP practice, supporting patients across chronic disease management, immunisations, screening, and preventative care.

We welcome candidates with COPD, Asthma, and Diabetes experience who are passionate about improving patient outcomes and promoting self-management of long-term conditions.

Qualifications

• Registered General Nurse with current NMC registration.
• Evidence of ongoing professional development.

Responsibilities

• Deliver Practice Nurse services within agreed competencies and NMC standards.
• Undertake chronic disease reviews including: Diabetes, Asthma, COPD.
• Conduct annual reviews and care planning for patients with long-term conditions.
• Administer vaccinations (childhood, adult, seasonal) in line with national guidance.
• Undertake cervical cytology screening.
• Provide lifestyle advice and support relating to smoking cessation, weight management, and healthy living.
• Maintain accurate patient records using EMIS Web.
• Participate in clinical audits and quality improvement initiatives.
• Work within NMC Code of Professional Conduct.
• Maintain professional registration and continuing professional development.
• Participate in mandatory training and practice meetings.
• Collaborate with GPs, HCAs, pharmacists, and the wider team.
• Support achievement of QOF targets.
