Job summary

Heaton Medical Centre is seeking an enthusiastic, well-organised, and experienced nurse to join our supportive team to provide high standard of service to our local population. The post-holder will work within patient group directions where available and deliver comprehensive care across long-term conditions, vaccinations, and health checks.

You will conduct health reviews, medications management, patient education, travel vaccinations, and blood test reporting within a training-practice

• Enthusiastic, well organised, experienced nurse.

Responsibilities

• Conduct patient health reviews for Long Term Conditions, development of individual patient centred management plans at annual review; agreeing these as appropriate with the patient and other health professionals in accordance with current guidelines and with knowledge of QOF requirements and NICE guidelines
• Performing medication reviews and assessments for long term conditions
• Educate, advise and support newly diagnosed and established chronic disease management patients
• NHS Health Checks/New Patient Checks
• Assist at IUD fitting clinics
• Testing Urine samples
• Blood results reporting (training and protocols provided)
• Assessment and administration of travel vaccinations
• B12 injections
• Depo administration
• ECGs
• Smears
• Pill checks/HRT checks
• Ordering & Administering appropriate immunisation and vaccinations
• Communicating with Child Health and D of H on immunisation queries and programmes
• Maintenance of cold chain and fridge temperatures
• Collaboration of equipment
• We are a training practice consisting of 7 GP partners, 2 Salaried GPs, 2 ANPs, GP Trainees, Paramedics, Pharmacists and Practice nurses. Our list size is approximately 11,000.
