Job summary

Putneymead Group Medical Practice is seeking an energetic Practice Nurse to join our friendly nursing team, consisting of an ANP, 3 Nurses and 3 HCAs. The post-holder will deliver high-quality nursing care to our patients, managing long-term conditions, delivering immunisations, supporting public health programmes, and promoting health and wellbeing across our local community.

Qualifications

• Registered Nurse with the NMC registration and license to practise.
• Competence in core nursing skills such as immunisations, wound care and cervical screening.
• Ability to work independently and within a team.
• Strong communication and interpersonal skills.
• Experience as a Practice Nurse and in management of long‑term conditions.
• Experience with audits, QA, and NHS primary care systems.

Responsibilities

• Deliver routine nursing care including wound care, ECGs, and health checks.
• Lead and support the management of long-term conditions (diabetes, COPD, asthma).
• Deliver immunisations, vaccinations and cervical screening programmes.
• Provide health promotion and prevention advice to patients.
• Support infection prevention and control IPC standards.
• Maintain accurate clinical records using appropriate coding systems.
• Participate in clinical audits, quality improvement, and service development.
• Work collaboratively within the MDT and wider PCN.
• Recognise clinical boundaries and refer to GPs or other professionals as needed.
• Support delivery of QOF targets and enhanced services.
