Qualifications

Responsibilities

• Develop and embed health promotion and well-being programmes.
• Manage chronic disease care plans and wound care.
• Support patients at risk of long-term conditions and refer as needed.
• Maintain accurate clinical records and read codes.
• Assist with minor procedures and vaccinations.
• Coordinate with external services and safeguarding.
