Job summary

Somerset LMC in Wells is seeking a motivated Practice Nurse to join our primary care team on a full-time basis (37.5 hours per week).

You will deliver high-quality nursing care including chronic disease management, immunisations, cervical screening, wound care, and health promotion, working closely with GPs and the wider MDT to improve local health outcomes.

Qualifications

• Registered Nurse with current NMC registration.
• Primary care experience desirable.
• Ability to work collaboratively in a multidisciplinary team.

Responsibilities

• Deliver high-quality nursing care to patients.
• Manage chronic diseases and promote health.
• Provide cervical screening, immunisations and wound care as part of routine treatments.
• Work with GPs and the wider MDT to improve health outcomes.
