Job summary

Watlington Medical Centre is seeking a Practice Nurse to deliver comprehensive general practice care and chronic disease management for our patient population. The role involves autonomous nursing duties, running chronic disease clinics and collaborating with the practice team to apply NICE guidance and local pathways.

Qualifications

• Registered General Nurse, currently registered with the NMC
• Educated to a good general standard
• Evidence of ongoing continuing professional development
• Knowledge of evidence-based management of chronic diseases such as diabetes, COPD, asthma and cardiovascular disease
• Working in accordance with the NMC Code of Professional Conduct

Responsibilities

• Assess, plan, implement and evaluate treatment programmes and care plans for long-term conditions
• Work with other health professionals to diagnose, monitor and manage long-term conditions
• Prioritise urgent or complex presentations and initiate emergency care when needed
• Deliver vaccination and immunisation programmes for adults and children
• Provide core general practice nursing procedures including wound care, cervical screening, contraception advice, travel health and phlebotomy
• Run structured clinics and annual reviews for chronic diseases in line with NICE guidance
• Initiate, titrate and review medicines for long-term conditions within your competence
• Support patients to self-manage their condition with home monitoring and action plans
• Maintain accurate long-term condition registers and contribute to QOF reporting
• Identify deterioration and escalate to GP or multidisciplinary team as appropriate
• Follow infection control, safety and safeguarding policies
• Record patient information accurately and maintain confidentiality
• Promote equality and diversity and support patient choice and chaperoning where needed
• Manage workload and time effectively
• Participate in audits and quality improvement activities
