Job summary

The Conduit is seeking an experienced Practice Nurse to deliver safe, autonomous patient care within a two-site GP practice. You will manage a diverse clinical workload, support audits, and contribute to local guidelines and care pathways.

As an NMC registered nurse with primary care experience, you will mentor colleagues, participate in clinical supervision, and help develop services such as chronic disease management and immunisations within the practice.

Qualifications

• NMC registration is essential.
• Experience in Primary Care is required.

Responsibilities

• Deliver safe, autonomous nursing care in a primary care setting.
• Mentor and support nursing staff in developing clinical skills.
• Participate in clinical supervision and routine audits.
• Maintain accurate patient records and ensure data privacy.
• Contribute to developing nursing services, guidelines and care pathways.
• Manage chronic disease programmes and immunisation delivery as part of the practice.
