Job summary

Whitstable Medical Practice is seeking a motivated Practice Nurse with experience in Chronic Disease Management to join a friendly multidisciplinary team across three sites in Whitstable, Kent. The role offers flexible hours (full-time or part-time) and a supportive environment to develop clinical skills within primary care.

The successful candidate will work across Estuary View Medical Centre, Chestfield Medical Centre and Whitstable Medical Centre on a flexible basis, contributing to asthma,

Qualifications

• Registered nurse with current NMC registration and competence in basic nursing duties.
• Experience managing chronic disease conditions in primary care.
• Able to work autonomously within professional scope and contribute to a multidisciplinary team.

Responsibilities

• Lead management and review of asthma, COPD and diabetes patients according to guidelines.
• Conduct comprehensive assessments and develop individualized care plans.
• Monitor disease control, adjust treatments, and review outcomes.
• Promote self-management and healthy lifestyle choices for patients.
• Collaborate with GPs, pharmacists and broader Practice team; refer appropriately.
• Maintain accurate clinical records and participate in audits and CPD.
