Qualifikationen

• Registered general nurse with current NMC registration and NHS primary care experience.
• Evidence of Chronic Disease Management and ongoing professional development.
• Good IT literacy and ability to use primary care systems.

Aufgaben

• Assess patients health needs and develop care plans.
• Screen and manage long-term conditions per practice protocols.
• Provide health education and support per national guidelines.
• Administer medications within PGD and prescribed instructions.
• Perform wound care, venepuncture, immunisations and related procedures.
• Collaborate with nurse team and other clinicians, maintain records.
