Qualifications

• Qualified Practice Nurse with NMC registration.
• Experience in long-term conditions management is desirable.

Responsibilities

• Carry out treatment room duties, chronic disease care, immunisations, health promotion and family planning.
• Work as part of the primary healthcare team providing Personal Medical Services using Systmone.
