Job summary

Chosen Care Group is seeking a PBS Lead to head Zero Harm initiatives for individuals with a learning disability, autism, or complex needs in East London, with visiting service users. You will own the PBS mission, guiding plans and staff actions to keep people safe and supported.

Responsibilities include driving FBA completion, staff training, and real-time data review to reduce incidents and remove restrictions, delivering measurable improvements in care quality.

Qualifications

• 5+ years in a PBS-focused role (LD, autism, or complex needs).
• Recognised Level 6 qualification in Positive Behaviour Support (PBS).
• UKSBA certified/registered.
• Experience delivering and leading PBS in Learning Disability and Autism services.
• Competency in Functional Behaviour Assessments.
• Knowledge of MCA 2005, DoLS & safeguarding.
• PMVA / MAPA / Studio3 training (or willingness).
• Genuine commitment to zero-incident, rights-based care.

• Equip staff with PBS plans and de-escalation strategies.
• Lead co-produced PBS plans with individuals, families, and MDT.
• Review plans within 48 hours and ensure staff can describe each plan.

WHAT YOU’LL BE ACCOUNTABLE FOR

• 1. Prevent incidents before they happen
• 100% of individuals have a completed FBA identifying triggers and proactive
• strategies
• All PBS plans include staged de-escalation protocols for staff to follow
• 50% reduction in incidents within 6 months — zero by month 12
• Target zero unplanned restrictions within 12 months
• Every restriction has a written elimination plan
• Full debrief and root-cause analysis within 5 days of any incident
• 3. Equip every staff member to prevent incidents
• 100% of staff trained before working unsupervised
• 90%+ of staff confident in de-escalation without restriction
• Zero incidents caused by staff not following a PBS plan
• 4. Make PBS plans genuinely effective
• All plans co-produced with the individual, family, and MDT
• Every plan reviewed within 48 hours of any incident
• 100% of staff can accurately describe the plan for each person they support
• 5. Use data to drive incidents to zero
• Real-time incident and near-miss data visible to the whole team
• Monthly reporting on trends, root causes, and prevention actions
• Near-misses recorded and acted on before they become incidents

ABOUT YOU ESSENTIAL

• 5+ years in a PBS-focused role (LD, autism, or complex needs)
• Recognised Level 6 qualification in Positive Behaviour Support (PBS)
• UKSBA certified/registered
• Relevant practical experience in delivering and leading PBS within Learning Disability and Autism services
• Competency in Functional Behaviour Assessments
• Knowledge of MCA 2005, DoLS & safeguarding
• PMVA / MAPA / Studio3 training (or willingness)
• Genuine commitment to zero-incident, rights-based care

DESIRABLE

• BILD PBS Competence Framework accreditation
• Experience in a CQC-registered service
• Knowledge of Makaton or AAC
