Our client in Essex are looking for 2 physiotherapist's for the below role;

Physiotherapist x2

Band 6 positions

8:30am - 4:30pm

Mon-Fri

Start Date 02/06/25

You will be responsible for undertaking assessment and intervention in the community and care homes for patients under the Discharge to Assess (D2A) pathways. Assessing patient's therapy and care needs, provide intervention to promote safety and independence , and aim to reduce the risk of re-admission or harm and to help support people onto the best pathway for their needs. You will coordinate services by undertaking a person-centred assessment; developing, implementing, and evaluating care and therapy plans, assessing equipment needs, supporting discharge planning and working on rehabilitation goals to promote function and independence. Post holder will work autonomously within professional guidelines and the overall framework of the service’s policies and procedures. Duties will include assessing for and health or social care packages to meet the patient’s needs. All these skills are competency led, and appropriate training will be provided where needed.

“Candidate needs to be willing to work across Mid and South Essex (however, if only one area can be covered, this can be possibly negotiated.

Due to the nature of the role, candidates will be required to hold a valid UK driving licence and access to their own car with appropriate insurance, as frequent travel in for visits will be require for this role”

Must have at least 6 months NHS experience in the UK
