Job summary

The Luton Falls team works across Luton aiming to prevent unnecessary hospital admissions following a fall, reduce the risk of further falls, and supports patients with falls prevention and health promotion. We work closely with the East of England Ambulance Service, local NHS services, GP practices, Social Services, and Luton Borough Council.

Well look no further! Our Luton Falls team is expanding, offering an exciting opportunity to join as a static Band 5 Physiotherapist.

Main duties of the job

The successful candidate will provide clinical support and timely interventions for individuals who have had a fall or are at risk of falling across Luton. Working with a multidisciplinary team, the role involves delivering physiotherapy services to enable patients to remain safely at home, prevent falls, and avoid unnecessary hospital admissions.

The role includes managing a clinical caseload, conducting evidence-based multifactorial falls assessments, and implementing interventions in community settings such as patients' homes, sheltered housing, and care homes. Supporting the delivery of strength and balance programs in the community is also essential.

Job responsibilities

Core responsibilities include:

  • Gaining valid, informed consent for assessments and treatments, in accordance with legal and professional guidelines, including cases where clients lack capacity.
  • Effective communication with patients, families, carers, and other professionals to maximise rehabilitation potential, using verbal, non-verbal, and written means.
  • Collaborating with healthcare professionals across sectors to provide seamless, coordinated care, adhering to trust guidelines for information sharing and consent.
  • Providing high-quality clinical care, supporting peers, and adhering to evidence-based practices and national guidelines.
  • Demonstrating compassion, empathy, and sensitivity, especially when dealing with complex emotional and physical conditions or delivering difficult news.
  • Undertaking comprehensive assessments, formulating diagnoses, developing treatment plans, and setting goals collaboratively with patients.
  • Prioritising appointments based on patient needs and delivering various therapeutic interventions within competence levels.
  • Providing advice and strategies for self-management, health promotion, and recurrence prevention, including accessible information and interpreters when needed.
  • Using objective outcome measures to evaluate and improve clinical practice.
  • Supporting community strength and balance programs, delivering educational talks, and following up post-program.
  • Managing risk assessments, equipment prescription, and safe handling of therapy equipment.
  • Traveling to community settings and working as a lone practitioner within policy guidelines.
  • Referring clients to outside agencies as needed and participating in public health initiatives like Falls Awareness Week.
  • Motivating clients and the public to participate in exercises and fall prevention activities.

Communication skills:

  • Effectively communicating complex and sensitive information, managing complaints, and ensuring confidentiality and appropriate information transfer.
  • Supporting partnership working across health, social, and voluntary sectors, and providing education and feedback to patients and colleagues.

Person Specification

Qualifications and Training
  • BSc, MSc, or Diploma in Physiotherapy
  • Current HCPC registration
  • Membership of the Chartered Society of Physiotherapy
Experience
  • Experience working within the NHS
  • Undergraduate placements in falls and frailty
  • Community experience, including facilitating exercise groups and prescribing exercises
  • Knowledge of a broad range of medical conditions
Knowledge and Skills
  • Time management and organisational skills
  • Car owner and driver
  • Understanding of clinical supervision and experience with SystmOne
