What you will do
- Collaborate as part of the Leaving Care Service in Peterborough to continually enhance and improve services for children, young people, and their families.
- Work alongside colleagues within Children's Social Care ensuring consistent practices across the county, fostering a supportive and collaborative culture.
- Partner with children, young people, parents, carers, and external agencies to deliver targeted support that improves outcomes within the community.
- Place young people leaving care at the heart of all practices and processes, ensuring timely support and resource optimisation.
- Collaboratively assess, plan, and intervene with care leavers and their families alongside a range of partner organisations.
- Manage a caseload in accordance with departmental policies, procedures, and relevant legislation.
- Utilise both manual and computerised systems to record confidential information and manage case records efficiently.
- Participate in and contribute to meetings, reviews, and panels across various settings including district councils and healthcare institutions.
- Adapt to user needs through flexible working, including lone working and unsocial hours when necessary.
- Safeguard vulnerable young people by maintaining awareness of relevant principles and communicating concerns to line management.
- Deliver services aligned with the Children Act Guidance particularly around transitions to adulthood for care leavers.
- Assess and support care leavers’ financial needs and assist with budgeting for accommodation and maintenance.
- Undertake assessments, data entry, reviews, and maintain detailed records in line with organisational policy.
- Engage in self and peer professional development activities through practice observations and structured supervision.
- Stay updated with the latest legislation, policies, and practises to deliver services effectively within delegated safeguarding frameworks.
- Prepare reports and documentation for various meetings and decision-making forums as needed.
- Plan and record Pathway Planning meetings to ensure young people receive appropriate guidance and support.
- Make home visits and attend meetings at various external locations as per organisational standards and procedures.
- Ensure service delivery is inclusive, addressing diversity and social justice issues.
