Job summary

This position supports young people leaving care and requires liaison with families, carers and external agencies to deliver targeted, effective services across Peterborough. A proactive, adaptable clinician will thrive in this dynamic role.

Responsibilities

• Collaborate with the Leaving Care Service in Peterborough to improve services for children, young people and families.
• Work with colleagues across Children's Social Care to ensure consistent practices across the county.
• Partner with children, young people, parents, carers and external agencies to deliver targeted support.
• Place young people leaving care at the heart of all practices and processes, ensuring timely support and resource optimisation.
• Collaboratively assess, plan, and intervene with care leavers and their families alongside partner organisations.
• Manage a caseload in accordance with departmental policies, procedures, and relevant legislation.
