Job Title: Personal Advisor
Job Purpose
To provide high-quality, person-centred support to Childrenin Care and Care Leavers, helping them successfully transition into independentadulthood. The Personal Advisor will hold and manage a caseload, develop andreview Pathway Plans, and provide practical, emotional, and developmentalsupport to enable young people to achieve positive outcomes in education,employment, housing, health, and wellbeing.
The post holder will work collaboratively with internalteams and external agencies to safeguard vulnerable young people, promoteindependence, build resilience, and ensure compliance with all statutory dutiesand responsibilities under relevant legislation and guidance.
Key Responsibilities
Case Management
- Managea caseload of Children in Care and Care Leavers, ensuring all statutoryresponsibilities are fulfilled.
- Develop,implement, monitor, and review Pathway Plans that reflect the individualneeds, aspirations, strengths, and risks of young people.
- Maintainregular contact with young people through home visits, meetings, andoutreach support.
- Buildpositive, trusting, and professional relationships with young people.
- Providepractical guidance and emotional support to help young people developindependent living skills, including budgeting, cooking, tenancymanagement, and accessing community resources.
- Supportyoung people in making informed decisions and achieving their personalgoals.
Safeguarding and Welfare
- Recognise,assess, and respond appropriately to safeguarding concerns.
- Followsafeguarding procedures and escalated concerns where required.
- Promotethe health, wellbeing, and safety of young people at all times.
Partnership Working
- Workclosely with social workers, housing providers, health professionals,education services, training providers, employers, and voluntaryorganisations.
- Coordinateand facilitate access to appropriate support services and interventions.
- Advocateon behalf of young people to ensure their needs are met.
Education, Employment and Training
- Supportyoung people in accessing and sustaining education, training,apprenticeships, and employment opportunities.
- Encouragepersonal development and career progression.
- Assistwith applications, interviews, and progression planning.
Record Keeping and Compliance
- Maintainaccurate, timely, and high-quality case records, reports, assessments, andcorrespondence.
- Ensurecompliance with relevant legislation, policies, procedures, andprofessional standards.
- Contributeto performance monitoring and service improvement activities.

- Promoteequality, diversity, and inclusion in all aspects of service delivery.

- Knowledgeof accommodation and housing pathways for young people.
Essential Qualifications
- Educatedto Level 3 qualification (A-Level, NVQ Level 3, Diploma, or equivalent) inHealth & Social Care, Youth Work, Social Care, Education, CommunityWork, or a related field.
Desirable Qualifications
- Level4 or Level 5 qualification in a relevant discipline.
- Professionalqualification in Youth Work, Social Care, Housing, or related area.
- Relevantcontinuing professional development relating to care leavers and youthsupport services.
Knowledge and Skills
- Knowledgeof legislation, guidance, and best practice relating to Children in Careand Care Leavers.
- Understandingof safeguarding principles and procedures.
- Excellentcommunication and interpersonal skills.
- Abilityto engage and motivate young people from diverse backgrounds.
- Strongorganisational skills and ability to manage competing priorities.
- Abilityto work independently and as part of a multidisciplinary team.
- CompetentIT skills and experience using case management systems.
- Knowledgeof housing legislation and welfare benefits.
- Understandingof mental health, substance misuse, and specialist support servicesavailable to young people.

- Abilityto travel within and outside the local authority area to undertake visitsand attend meetings.

WorkingArrangements
- Hybrid Working: Therole is office-based to support the team's duty function, with regularvisits to care leavers and office attendance as required.
- Travel Requirements: Thepost holder will be required to undertake home visits, with the majoritytaking place within Nottingham City and the surrounding local area.
