Job summary

NHS Jobs is seeking a Peer Support Worker for Drug & Alcohol Harm Reduction and Rough Sleeping to provide peer support to people affected by substance use and homelessness. You will share lived experience to inspire hope, help clients set goals and access healthcare, housing and treatment, and promote self-determination in a community setting.

You will work with a multidisciplinary team to support harm reduction and safety, contribute to care planning, risk monitoring and outreach, and

Qualifications

• Provide peer support to people affected by drug and alcohol use and homelessness, using lived experience to inspire recovery.
• Assist service users to identify goals, strengths and opportunities for positive change.
• Support access to healthcare, housing, treatment and community resources; promote self-determination.

Responsibilities

• Provide peer support to clients in community settings, focusing on harm reduction and recovery.
• Contribute to care planning, risk monitoring and communication with the multidisciplinary team.
• Participate in outreach, supervision, training and service development initiatives.
