Join a dynamic team as a Health Care Assistant (HCA) in a thriving Primary Care Network. This role balances practice work with varied PCN responsibilities, requiring mobility among member practices. This is an unusual opportunity to join a mature and well developed Primary Care Network (PCN) in an adapted split role. We are seeking an HCA (Primary Care Support Worker) to be employed by West Leeds PCN but mainly deployed at Hillfoot Surgery, one of our member practices. The successful applicant would work for 80% of their working week at Hillfoot doing the practice work they require; the remaining 20% will be PCN work which will cover all of our 6 member practices. This role will be varied and interesting and will require moving around between all of our member practices. Therefore, a driving licence and access to a car will be crucial. The workflow elements of this role will be predominantly as you would expect from a practice based HCA but with the added variety of some PCN work. Please see the uploaded job description for more information about the responsibilities of the role
West Leeds Primary Care Network (PCN) has a significant workforce of around 60 people in varying clinical disciplines. The PCNalready employs a significantHCA workforce which works at scale on a pro-rated basis across all of our sixmember practices. The PCN HCA's very much form the backbone of a great deal of our clinical work so it is a vibrant team to be part of. Hillfoot Surgery, the main base for this role has GP partners, a nursing and HCA team plus a full administrative team on site to help you bed in to this slightly unusual role. - ECG test (but not thereading and assessing of the results)

- Urinalysis
- Height and weightrecording
-Physical checks forpatients with serious mental illness (SMI) and learning disabilities (LD)
- Dressings
- Supporting GPs andpractice nurses with long term condition reviews
- Health promotion andsignposting to services
- Processing laboratorysamples
- Vaccine / cold chainstorage, monitoring and recording
- Ordering of medicalequipment
- Surgical equipmentrestocking / stock rotation
- Clearing and restockingconsultation rooms
