An NHS opportunity for an Enhanced Nurse within the SWAN PCN to improve health outcomes and reduce inequalities across Wigan. This role includes providing clinical leadership, mentorship, and support to various health teams, emphasizing proactive, personalized care and community engagement.
An exciting opportunity has arisen withinSWAN Primary Care Network (PCN) to recruit an Enhanced Nurse to play a centralrole in our drive to improve health outcomes and reduce inequalities across ourneighbourhood. This role is ideal for a registered nurse with a passion forproactive, personalised care, collaborative working, and developing the futureworkforce.
Working across our networks practices and communities,you will provide clinical leadership and mentorship, contribute to populationhealth initiatives, and support our Care Home & Frailty team. You will be akey part of our neighbourhood-based, multidisciplinary approach to healthcaretransformation.
Key Clinical Areas
Tackling Health Inequalities and delivering proactive neighbourhood care
Providing mentorship and supervision to trainee roles within the PCN team
Supporting our Care Home and Frailty teams through holistic, person-centred care
Leading on Infection Control, Clinical Governance, and Policy Development
SWAN Primary Care Network constitutes 7 GP practices, located in South Wigan Ashton North area, with a collective patient population of around 35,500. The member practices are Marus Bridge Practice, Medicentre, Bryn Cross Surgery, South Wigan Medical Practice, Winstanley Medical Centre, The Grange Practice and Kumar Family Practice.
We believe we are the optimum size to enable us to work effectively together, maintain our practice identity and improve patient services and health outcomes. The vision for SWAN PCN is to collaborate with the local community to provide a wide range of services tailored to the need of our patients.

Main responsibilities of the PCN nurseinclude the following areas:

Keymember of the PCNs Community Steering Group
Buildpartnerships across primary care, community services, local authority, andvoluntary sectors
Engagewith underserved communities to co-produce solutions and support equity ofaccess
Design,deliver and evaluate programmes to reduce health inequalities
Leadproactive clinics in the community using a population health approach
Userisk stratification and data to target interventions effectively
Provideclinical leadership at community-based and neighbourhood clinics
Mentorship and Clinical Supervision
Mentorand supervise PCN trainee roles (e.g., trainee nurses, nursing associates, GP assistants)
Supporttheir clinical development and professional growth
Coordinatelearning opportunities including training, shadowing, and case-baseddiscussions
Provideregular feedback, conduct appraisals and contribute to skill development
Promotea culture of learning, reflection, and continuous improvement
Care Home and Frailty Support
Providecomprehensive health assessments and care planning for frail and houseboundpatients
Participatein multidisciplinary ward rounds in care homes
Offerproactive support for palliative care, dementia, and complex long-termconditions
Supportpatients and carers in managing conditions and navigating services
Deliveropportunistic health promotion and wellbeing advice
Leador contribute to the development of personalised care and support plans
Maintaineffective communication with wider clinical teams and families
Infection Control, Governance &Policy Management
Actas PCN Infection Prevention and Control (IPC) Lead
OverseeIPC audits, training and policy compliance across the PCN
Develop,implement and review clinical protocols and policies
Leadand participate in clinical audits and quality improvement initiatives
Supportshared learning following significant events or near misses
Respondto national or local policy changes affecting clinical delivery
Other
Tomaintain accurate clinical records in conjunction with extant legislation
Theremay, on occasion, be a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels

This roleprofile is intended to provide a broad outline of the main responsibilitiesonly. The post holder will be expected to be flexible in relation to dutiesundertaken and location of work where required by the PCN.
Musthave access to own transport and be able to travel across sites and into thecommunity.
Flexibilityto work outside of core hours where required.
