Job summary

The Methodist Church in Britain is seeking a fixed-term community nurse to support five GP practices within the WHA South Primary Care Network. You will assess patient needs, deliver nursing interventions and support GPs, with enhanced care for frail and elderly patients in care homes.

Initially, the post involves close supervision, flexible work across surgeries and community settings, and a focus on long-term conditions and dementia through the CaCHT team.

Qualifications

• Registered General Nurse.
• NMC registration as a general nurse.

Responsibilities

• Provide patient-facing clinical care, advice and support, initially under close supervision.
• Assess patient needs, identify risks and deliver nursing interventions.
• Support GPs and the wider primary healthcare team in delivering high-quality care.
• Deliver PCN DES and Enhanced Health in Care Homes services under the Lead PCN Nurse's guidance.
• Perform routine patient care tasks as directed by senior nursing staff and GPs.
• Chaperone and assist patients being examined by another clinician.
• Request pathology tests, such as urine cultures and swabs.
• Follow agreed clinical protocols, referring to GPs as appropriate.
• Work within a multidisciplinary team, guiding colleagues such as Care Coordinators and Health Coaches.
• Respond flexibly to the needs of patients, carers and the service within scope of practice.
• Work across the PCN footprint, including other surgeries and community settings.
• Contribute to developing and expanding PCN nursing services and service improvement.
