Job summary

Operose Health is recruiting a PCN Care Coordinator to join the Nottingham-based team. 37.5 hours per week, delivering front-line patient access, handling appointment requests, signposting services and maintaining confidential records.

You should have reception or customer care experience, familiarity with SystmOne, and solid PC skills (Word, Excel, email). NHS exposure is welcome but not essential.

Qualifications

• Reception or customer care experience is essential.
• Experience with SystmOne is required.
• Must have basic PC skills such as Word, Excel and email.

Responsibilities

• Responding to patient queries and liaising with the wider primary care team; manage appointment requests and signpost patients to services; maintain patient records and confidentiality.
• Emailing, scanning and coding clinical correspondence.
• Processing prescriptions requests.
• Utilising other information systems to support efficient workflow processes.
